USOD Fall 2011 Family Grant 

Overview

Family Grants are made available to the membership of The Up Side of Downs to provide financial assistance which directly improves the life of the individual with Down syndrome. The funding for Family Grants comes from donations and fund raisers. Grants are awarded based on the availability of funds, which varies based on the amount of donations and participation in fund raisers in a given year.

Eligibility

Grants are NOT based on the financial status of the family, and will be awarded once per calendar year per individual with Down syndrome. Receipt of a grant in one year, does not guarantee receipt of a grant the following year. Must be a resident of Cuyahoga, Geauga, Lake, Lorain, Medina, Portage, Stark or Summit Counties. Priority is given to active members of USOD and volunteer hours are encouraged.

Grant Amounts

Financial assistance is awarded for recreational activities such as dance classes, team sports, or swimming for up to $300. Direct medical expenses can be reimbursed for up to $600.  All grants are awarded in the form of checks only payable to the service provider (e.g. summer camp, speech therapist, medical supply store, etc.)

Application Process

To apply for a Family Grant, please complete the attached application: USOD_Family_Grants_Fall_2011_Final_distributed.pdf.  Feel free to include an explanation of any unusual/special circumstances regarding your need that may not be evident by simply completing the application form. Once your application is received, the Grant Committee will review it and determine if the applicant is eligible. Final decisions will be made after the deadline of October 31, 2011.

The Grant Committee will notify the family in writing with an explanation of the decision. If a favorable determination is made, USOD will write a check to the service provider indicated on the application. All grants will be mailed to the family; however the check must be paid only to the order of the provider.
